SUMMARY:Vision Clinic Exam & Eyeglasses- Colbert Location
DESCRIPTION:In partnership with The Lighthouse\, MedLink Georgia will be hosting a Vision Clinic for exams and eyeglasses.\n\nAll glasses start at $15 for bifocal and single vision prescriptions (mailing is included in this fee). If the prescription is over +/-3.00\, polycarbonate material is required\, which is an additional $15. If no-line bifocal (progressive) is preferred that starts at $65. Glasses range from $15-$65 depending on the prescription.\n\nPAYMENT POLICY\n\n  Payment is due on the day of the clinic.\n\n  Patient has until the end of the clinic day to submit full payment for eyeglasses.\n\n  If patient cannot make full payment for eyeglasses before the end of the clinic day\, then patient will have to wait\n\nuntil the next clinic date to submit full payment and be refitted for frames.\n\n  Eyeglasses will NOT be placed on hold for patients.\n\nWARRANTY\n\nWarranties are NOT provided for lost\, stolen\, or broken eyeglasses. Full price according to the "Additional Pair" price\n\nbracket is required for replacement pairs.\n\nPatients are allowed up to 3 PAIRS MAXIMUM (including first pair) (NO\n\nEXCEPTIONS) during their 2 year eligibility period.\n\n-If the frame is damaged with the lenses intact\, and the Lighthouse has another frame of that style and size in stock\, the\n\npatient may replace their frame for a $20 fee.\n\n  Remakes that are a result of a doctor's RX change by a Lighthouse affiliated doctor\, are free 1 time only within\n\nthe first 90 days of receiving eyeglasses. Those RX changes coming from outside providers\, including Grady\n\naffiliates\, require full price for a remake.\n\n  Remakes that are a result of a lab error or a clinical fitting/measurement error are free within the first 90 days\n\nof receiving eyeglasses.\n\n  After 90 day period ends\, full price of lenses and upgrades will be required for a remake regardless of reason.\n\nELIGIBILITY REQUIREMENTS\n\n*Patient must be a GA resident for last 12 months.\n\n*Patient must meet the sliding fee scale requirements. Insurance does not matter for glasses\; if a patient has coverage for eye exam with insurance\, they may bring their prescription in for glasses purchase if they qualify for slide.
